For both new and established patients — use this page as your central reference before every visit.

Quick checklist (skim)

  • My Account: Photo current; contact, insurance, and payment verified.
  • Forms: Complete outstanding questionnaires in the portal.
  • Meds & Supplements: Update prescriptions, OTCs, vitamins/herbs; upload bottle photo if unsure.
  • Labs & Documents: Upload recent/past labs, imaging, and records (fax/email ok).
  • Allergies & Intolerances: List all allergies and sensitivities.
  • Vitals: Update height and weight.
  • Pharmacy: Confirm preferred pharmacy.

1) MY ACCOUNT

  • Upload a recent photo if you haven’t already.
  • Add, update, or verify your contact information, insurance details, and preferred payment method.
  • Prescriptions and lab tests often require insurance approval. If the plan we have on file is out of date, delays are likely—keeping your information current helps everything run smoothly.

2) ESSENTIAL QUESTIONNAIRES & FORMS

  • Complete and sign any outstanding documents in the portal.
  • Review these forms before every visit—whether you’re a new or returning patient—as our policies and details may change over time.

3) MEDICATIONS & SUPPLEMENTS

  • Add or update all prescription and over-the-counter medications you’re taking (e.g., allergy meds, antacids, ibuprofen).
  • Add or update any vitamins, supplements, herbs, or protein powders you’re using.
  • Even small differences matter—if you can’t find an exact match, upload a photo of the bottle instead.

4) LABS & DOCUMENTS

  • Upload any recent or past labs, imaging, or medical records.
  • Not sure if that lab from 1985 is still relevant? Send it along—the more information we have, the better we can understand your full health story.
  • You may upload directly to the portal, fax, or email if that’s easier.

5) ALLERGIES & INTOLERANCES

  • List all allergies and intolerances.
  • Whether it’s cilantro tasting like soap, milkshakes that don’t agree with you, or mosquito bites that linger for days—every detail helps us personalize your care.

6) VITALS

  • Update your height and weight in the portal.
  • Some medications and supplements are dosed by weight, so accurate numbers help us get it right.

7) PHARMACY

  • Confirm your preferred pharmacy.
  • Spending time on your health is more valuable than tracking down whether your pharmacy is on Main Street… or Maine Street. Updating this now saves stress later.
